Sign your Deed Poll
When you receive your Deed Poll document, you will need to sign and date it in the presence of an independent witness.
Your witness could, for example, be a friend, neighbour or work colleague who is independent of your application.
What happens next?
Once your Deed Poll has been correctly signed and witnessed, it can be used as documentary evidence of your change of name.
You can then contact the government departments, companies and organisations that hold your personal records and ask them to update your name.
This may include organisations such as HM Passport Office, DVLA, banks, employers, pension providers, utility companies, GP surgeries and other organisations that hold records in your previous name.
